Your login is always your last name and the last four digits of your social security #.  

If you change your name, Doculivery will not automatically change it.  Please contact Payroll to make the change if you wish.

The first time you ever login, your password is the last 4 digits of your social security #.  You will then create your own password and security questions to use going forward.  If you forget your password, click on "forgotten password" to answer the security questions created by you and you will then be prompted to reset your own password.
